In a facial serum, hyaluronic acid is often formulated with lower molecular weights to penetrate deeper layers. In a hand cream, it typically stays on the upper stratum corneum, where it reduces the tight, parched feeling that follows washing or cold exposure. The effect is temporary — it lasts until the next hand wash or until the cream wears off — so reapplication is what maintains the smoother, more supple feel.
